 To apply for a debt consolidation loan you’re going to need to bring paperwork including a copy of your income and budget for each month so the lender can decide whether or not you can afford to repay the loan.  This is much more convenient for most people and allows you to only worry about a single bill each month.  Then you can focus more on your finances and making sure that you’re on time with payments.  It’s important to have good credit and even after going through a horrible crime like identity theft you need to try and get back on track.
